Pemi-Baker Hospice & Home Health extends its sincere thanks to the Grafton County Pemi-Baker Centennial Lions Club for a generous $1,000 donation in recognition of Pemi-Baker’s ongoing work to raise awareness about diabetes and support local families living with the disease.
Diabetes impacts more than 37 million Americans — that’s about 1 in every 10 people — and nearly 1 in 5 don’t even know they have it. In Grafton County, diabetes is a growing challenge for many families who rely on community health services to manage this chronic condition. Without proper management, diabetes can lead to serious health issues like heart disease, kidney damage, nerve damage, and vision loss.
Through skilled nursing, home health care, diabetes education, and hospice services, Pemi-Baker’s dedicated team helps patients monitor blood sugar levels, manage medications, prevent complications, and understand lifestyle changes that can improve quality of life.
