Have you ever wondered how to talk to your loved ones about what medical care you would—or wouldn’t—want in a medical crisis? Has your health care provider encouraged you to have these important conversations and document your wishes?
In recognition of Health Care Decision Day, Pemi-Baker Hospice & Home Health, in partnership with the Grafton County Senior Center, is offering two free Advance Directive workshops to support individuals in planning for future health care decisions.
Advance care planning is important for every adult age 18 and older. A serious illness or unexpected accident can leave anyone unable to communicate their wishes. This can happen to young adults, parents, or otherwise healthy individuals—such as someone involved in a car accident, experiencing a sudden medical emergency like a stroke, or facing complications during surgery. Having an advance directive in place ensures your voice is heard, even if you cannot speak for yourself.
These workshops are designed to guide participants through the process of advance care planning in a supportive, neutral environment. Attendees will have the opportunity to reflect on and identify the personal values, beliefs, and goals that shape their health care decisions. An experienced facilitator will explain the importance of sharing these wishes with others, ensuring that decisions can be honored if an individual is ever unable to speak for themselves.
Participants will also receive a free Advance Directive form and assistance in completing it.
