Ways to make a gift

PBH&HH depends on the generosity of people from all walks of life to support our life-changing programs and services. Contributions help us fulfill our mission to provide home care and community services to all Granite Staters, regardless of their financial circumstances.  When you donate to PBH&HH, your gift helps to ensure that the familiar knock of the Pemi-Baker visiting nurse will be heard at the door of a NH area family who needs us.

Make a Gift Today

It’s easy to change the life of one of our vulnerable neighbors. To make a one-time or recurring donation, fill out our donation form on line by scrolling down or download our donation form if you would rather mail it in.

You can choose to direct your contribution to a specific program or area of need, and make a gift in honor or in memory of a loved one or a special Pemi-Baker staff member.

Estate Gifts

You may wish to include PBH&HH in your will, or make a tax-savvy gift such as an IRA, trust, retirement plan, or life insurance.  All gifts will help us bring professional, skilled and compassionate medical care to those in need, continuing our charitable work for generations to come. You can creatively craft your gift to maximize the impact on our patients and potentially create tax benefits for you and your family.

  • Bequests

Making a gift by bequest to PBH&HH can be as simple as inserting a paragraph into your will. See the sample language below.

I give, devise and bequeath to Pemi-Baker Hospice & Home Health, with offices at 101 Boulder Point Drive, Suite 3, Plymouth, NH 03264 and federal tax ID 02-0273178, or its successors in interest, exclusive of my lifetime giving, if any, the sum of $____, to be used for its general purposes at the sole discretion of its trustees.

Your attorney can change this to a percentage of your residual estate or a specific asset, or to specify a particular program.

  • Gifts of Retirement Assets & Life Insurance

You can help bring much needed home-and community-based care to the lives of your neighbors simply by naming PBH&HH as a beneficiary of a life insurance policy, IRA, 401(k), 403(b), pension, or other retirement plan. I was a mess after having a triple bypass BUT the people at Pemi-Baker helped me so much. Each one were kind, caring, helpful and very sincere with their care. I am so grateful for this service. The physical therapists who visited after my total knee replacement were amazing! The communication was excellent. They phoned in advance to confirm their arrival times. The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
