Pemi-Baker Hospice & Home Health Offers Complex Care Support for Common Serious Illnesses of Aging. 
As people age, many begin to experience chronic health conditions that can make everyday life more complicated. Illnesses such as cancer, heart disease, Alzheimer’s disease, COPD, kidney disease, stroke, and diabetes are increasingly common among older adults, and managing them often requires ongoing care, education, and support.
To help patients and families navigate these challenges, Pemi-Baker Hospice & Home Health offers its Complex Care Support Program, designed to provide additional assistance to individuals living with serious illnesses while they continue receiving active medical treatment.
For many families, managing a serious health condition can quickly become overwhelming. Multiple doctor appointments, medications, changing symptoms, and uncertainty about what lies ahead can create stress for both patients and caregivers.

The program focuses on education, care planning, and resource navigation, helping patients better understand their illness and the options available to them.
Pemi-Baker’s skilled home health team includes experienced registered nurses, licensed nursing assistants (LNAs), physical therapists, and occupational therapists who work together to support patients in the comfort of their homes.
Services through the Complex Care Support Program may include:
- Nursing visits to monitor symptoms and help manage complex medical needs
- Medication education and management to ensure treatments are safe and effective
- Physical and occupational therapy to maintain strength, mobility, and independence
- Personal care assistance provided by compassionate licensed nursing assistants
- Education for patients and caregivers so families feel prepared and informed
- Care planning and guidance to help patients and families understand what to expect in the future
By offering care in the home, the program helps individuals remain in familiar surroundings while receiving the support they need to maintain the best possible quality of life.
A key part of the Complex Care Support Program is collaboration with existing healthcare providers. The Pemi-Baker team works in conjunction with physicians and specialists, enhancing the care patients are already receiving and ensuring communication between all members of the care team.
The program also offers nurses on-call 24 hours a day to assist with urgent needs, providing reassurance that help is always available if questions or concerns arise.
