A Safe Space for Healing: Grief & Loss Support Group

January 21, 2026

A Safe Space for Healing: Grief and Loss Support Group at Plymouth Senior Center.Free Grief & Loss Support groups offered at Pemi-Baker Hospice & Home Health, Plymouth, NH

The experience of grief that follows upon a loss is a deeply personal and highly individual reality.  Its characteristics can be shaped and formed by the attachment we had to what was lost and the nature of the event of the loss itself.  The loss of an abusive parent we reluctantly assumed responsibility caring for as they approached the end of their life may arouse different feeling responses than the sudden death of a much-beloved pet who brought great joy into our life.  How we come to terms with the bundle of feelings that make up our own unique experience of grief is the challenge of mourning that can bring healing.

The next Grief and Loss Support Group offered by Pemi-Baker Hospice & Home Health will attempt to assist participants in looking at the particular features of their grief.  There is no predictability regarding how we respond to loss, the types of reactions we may have, the frequency or intensity of these, or how long it will take to “feel better” or “get back to normal”.  The group is intended to provide a safe space where participants can share their experiences and questions, be heard, and listen to others.  Hopefully, it can help to recover some balance and stability if the emotional reactivity feels confusing and stormy.

Our first of six sessions in this series will be on Tuesday, February 3, 2026, at the Senior Center in Plymouth, 10:30 AM.  Since we are in the winter months, when the weather is bad, we ask participants to check WMUR Channel 9 for closings.  When the Plymouth Schools are closed, the Senior Center is also usually closed as well, so we will not be meeting.  Even if schools are open when the weather is bad, we ask participants to use their best judgment in doing what they need to do to remain safe.

Sessions will be held in the upstairs art room of the Plymouth Regional Senior Center (8 Depot Street, Plymouth), accessible by elevator. Participants needing a parking slip should plan to arrive by 10:15 AM to obtain one at the front desk.

Free Workshop: Wellness, Health & Happiness as We Age

January 15, 2026

Pemi-Baker Hospice & Home Health invites community members to attend a free educational workshop, “Wellness, Health & Happiness as We Age,” on Thursday, January 22nd from 6:00–7:00 pm at the Waterville Valley Recreation Department, Multipurpose Room, located at 11 Noon Peak Road, Waterville Valley, NH.

The workshop will be led by Dr. Kristi Saunders, Hospice Medical Director at Pemi-Baker Hospice & Home Health, who will explore how true health is more than the absence of illness—it also includes physical, mental, and spiritual well-being. The discussion will focus on aging well, maintaining balance, and finding joy and meaning at every stage of life.

Dr. Saunders and Gretchen Smith, RN, will also explain the important differences between an Advance Directive and a POLST (Physician Orders for Life-Sustaining Treatment) form. Participants will learn the advantages and benefits of each and how these tools help clearly communicate personal healthcare wishes.

An open Q & A session will follow, providing attendees the opportunity to ask questions and engage in meaningful conversations about aging, wellness, and living a joyful life.

This event is free and open to the public. All are welcome.

For more information about this workshop or other educational programs, please contact Pemi-Baker Hospice & Home Health: 603-536-2232.

Hospice Care Offers More Help and Hope Than Many Realize

December 10, 2025

Facing a terminal diagnosis is one of the hardest moments for patients and the people who love them. Pemi-Baker Hospice & Home Health wants families to know that hospice care can bring comfort, guidance, and a stronger focus on quality of life during this time. You matter, Hospice care from Pemi-Baker Hospice & Home Health

Many people believe that starting hospice means death is near, but that is not always the case. Some patients even improve enough to graduate from hospice care. Pemi-Baker hopes to clear up common misunderstandings and help families see how much support hospice can offer.

To receive hospice services, a patient must have a terminal diagnosis with an expected life expectancy of six months or less. This is an estimate, not a deadline. Pemi-Baker encourages families to call as soon as they receive a diagnosis so a hospice consult can be done in the home. This gives the entire family a chance to ask questions, learn about available services, and find out if the patient qualifies. Hospice is covered under Medicare A benefits, which means the hospice provider pays for care related to the terminal diagnosis. The hospice care team at Pemi-Baker Hospice & Home Health, Plymouth, NH

Hospice care includes a wide range of services. These may involve skilled nursing visits, medical equipment, supplies, medications, social worker support, chaplain services, home health aides, bereavement support for families, and respite care. All services can be provided in the patient’s home or in a facility.

For patients living in a facility, hospice adds an extra layer of support on top of the care already being provided. This helps ensure the patient’s physical, emotional, and spiritual needs are met in a well-coordinated and personal way.

“Hospice is not about giving up. It is about helping people live their remaining time with as much comfort and dignity as possible, while also supporting the families who walk this path with them,” said Aubrey Engle, RN, Executive Director of Pemi-Baker Hospice & Home Health.

Pemi-Baker Hospice & Home Health hopes more families will explore hospice care as a positive and supportive option during a difficult stage of life.

Don’t wait to ask about how our compassionate staff can help your loved ones with the Gift of Hospice Care. To qualify for hospice care patients, families and providers can refer to the program if the patient’s condition is life limiting and the patient is seeking symptom management support at home or in a nursing home/hospital. Our focus is on providing comfort to patients and support to families, with our team available 24/7 to address any concerns. The call is free, and our professionals are ready to assist you! Guidance for a Smooth Surgical Recovery at Home

November 26, 2025

As many in our community prepare for scheduled surgeries this winter and into the new year, Pemi-Baker Hospice & Home Health is raising awareness about the importance of planning ahead and utilizing home health services to ensure a safe, successful recovery. Whether undergoing joint replacement, cardiac surgery, abdominal procedures, or other medically necessary operations, thoughtful preparation and professional support can significantly reduce complications and speed the return to daily life.

Preparing Before Surgery: Small Steps, Big Benefits

When a surgery is planned, patients have a valuable opportunity to set themselves up for a smoother, safer recovery. Pemi-Baker encourages individuals and their caregivers to consider the following preparation strategies:

A home healthcare worker helping a senior man with a walker walking through the front door of his house.

  • Home Safety Check: Remove loose rugs, improve lighting, and create clear walkways. If mobility will be limited, set up a temporary living space on one floor to avoid stairs.
  • Support System Planning: Identify a family member, friend, or neighbor who can help with meals, errands, and transportation during the first days or weeks post-surgery.
  • Medical Readiness: Fill prescribed medications in advance, prepare ice packs or wound supplies if appropriate, and schedule follow-up appointments before the procedure.
  • Equipment Prep: Items like walkers, shower chairs, raised toilet seats, or reachers may be recommended. Obtaining these ahead of time can prevent unnecessary strain or falls.
  • Home Health Referral: Ask your surgeon or primary care provider about a home health referral to Pemi-Baker Hospice & Home Health. Having this in place before surgery ensures services can begin promptly once you return home.

These simple steps can ease stress, support healing, and reduce the likelihood of setbacks after discharge.

Expert Care at Home: What Pemi-Baker’s Team Provides

Following surgery, Pemi-Baker Hospice & Home Health delivers skilled, individualized care designed to help patients regain strength, mobility, and independence:

  • Registered Nurses (RNs): Nurses monitor vital signs, manage wound care, review medications, watch for infection or complications, and communicate closely with the patient’s physician.
  • Licensed Nursing Assistants (LNAs): LNAs assist with personal care such as bathing, dressing, and safe transfers, helping patients maintain comfort and dignity while healing.
  • Physical Therapists: PTs develop customized exercise and mobility plans to restore strength and balance, reduce pain, and support a safe return to walking and daily activities.
  • Occupational Therapists: OTs help patients relearn or adapt everyday tasks—like cooking, bathing, or getting in and out of bed—while recommending adaptive equipment and home modifications to improve independence.

Together, this interdisciplinary team offers hands-on care, education, and encouragement, empowering patients to progress confidently at home rather than struggling alone or returning to the hospital.Shower modifications for patients, Pemi-Baker Hospice & Home Health

Proven Results: Reduced Falls and Fewer Hospital Readmissions

Research consistently shows the benefits of home health services after surgery. According to national Medicare data, patients who receive home health care experience up to a 25% reduction in hospital readmissions compared to those without post-acute support. Additionally, structured physical therapy and home safety interventions can reduce fall risk by nearly 30%, a critical factor since falls are one of the leading causes of injury and rehospitalization for older adults after surgery.

By providing early mobility guidance, medication oversight, and vigilant monitoring, home health services help patients recover more safely—right in the comfort of their own home.

A Partner in Healing

“Our goal is to give patients the confidence, education, and support they need to heal well,” said Gretchen Smith, RN, Clinical Manager. “The transition home can feel overwhelming, but with the right preparation and compassionate professional care, recovery can be smoother, safer, and far less stressful.”

Pemi-Baker Hospice & Home Health encourages anyone anticipating surgery—or caring for a loved one who is—to speak with their healthcare provider about a home health referral.

Don’t wait to ask about how our compassionate staff can help your loved ones. To qualify for home health care, a person must have a physician’s order and demonstrate a need for skilled care that can be safely provided at home. Our focus is on providing comfort to patients and support to families, with our team available 24/7 to address any concerns. The call is free, and our professionals are ready to assist you! Thanksgiving Gratitude & Invitation to Join Circle of Friends

November 25, 2025

Pemi-Baker Hospice & Home Health Shares Thanksgiving Gratitude and Invites the Community to Join Its Circle of Friends.

As Thanksgiving approaches, Pemi-Baker Hospice & Home Health extends heartfelt thanks to the people who make our mission possible. This season reminds us to pause, look around, and appreciate the strength of a community that continues to show up for one another. Your support, kindness and trust allow us to bring healing, hope and care to friends, neighbors and families throughout the region. For that, we are truly grateful.Pemi-Baker Hospice & Home Health feeling Thankful

We also want to shine a light on the people who carry out this work every day. Our nurses, therapists, social workers, aides, administrative staff and volunteers give their energy, skill and compassion to every visit and every call. They stay late. They listen closely. They guide families through some of the hardest moments they will ever face. Their dedication is steady and sincere, and we thank each member of our team for standing strong for our community.

This Thanksgiving, as many gather around full tables, we recognize that our work continues in living rooms, care facilities and homes where illness, recovery or loss reshapes family routines. Because of your support, our team can bring comfort, safety and clarity to these moments. From helping someone return home after surgery to supporting a family through hospice care, your generosity fuels the services that matter most.

As the holiday season begins, we also want to remind our community that Giving Tuesday is December 2nd. This global day of generosity gives each of us a chance to invest in the wellbeing of our neighbors. If you have ever wondered how to make a meaningful difference with a small budget, we invite you to join Pemi’s Circle of Friends.

The Circle of Friends is a growing group of monthly donors who believe that no one should be denied care based on their ability to pay. Even the smallest monthly gift becomes steady, reliable support that allows us to mobilize care when families need it most.Pemi-Baker Hospice & Home Health Circle of Friends monthly giving club

Here is what a monthly commitment can do:

  • Five dollars helps our chaplain, Guy Tillson, connect with ten families who need support after a loss or during the strain of caregiving.
  • Ten dollars helps coordinate equipment and services for someone returning home after surgery.
  • Twenty dollars covers foot and blood pressure screenings for seniors, giving them a simple but important check on their health.
  • Thirty dollars helps our team provide palliative care for people living with chronic illness. This work brings comfort and guidance to families facing difficult diagnoses.
  • Fifty dollars helps us support families caring for a loved one on hospice when services extend beyond what Medicare or insurance will cover. Our social workers play a crucial role during these moments, and your gift helps keep their support available.

A monthly gift of any amount creates impact all year long. It keeps families steady during times of fear or uncertainty. It helps our team respond quickly and thoroughly. It ensures that no one feels alone at a time when connection matters most.

This Thanksgiving, we thank you for believing in our mission. We invite you to carry that spirit forward by joining our Circle of Friends and supporting care that reaches every corner of our community.

Hospice care with Pemi-Baker Hospice & Home Health, Plymouth, NH

“I would give your hospice team 10 stars if I could!”

They were truly amazing. We called on a Sunday, and they sent someone out that very afternoon to begin care and stayed by our side as Mom's health declined. We are especially grateful for Alexa, who went above and beyond to attend to our mom's needs and support us as caregivers. She was exceptionally knowledgeable, compassionate and simply an incredible human to be around. We couldn't have done this without her and will be forever grateful for her care. We also want to acknowledge Gretchen and Dr. Saunders, who guided us through some very challenging moments and took the time to truly listen, support us and answer all of our questions. I cant' say enough about our experience with hospice. We are deeply grateful for their care, compassion and support during this time.

I will forever be grateful for your support, wisdom, humor and kindness, all along the end of life journey for mom. I am so deeply thankful that circumstances conspired such that mom was able to die at home, and your role made that time as seamless as it could be made.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
