Celebrating National Nurses Week, May 6–12th.

April 30, 2026

Pemi-Baker Hospice & Home Health proudly joins healthcare organizations across the country in celebrating National Nurses Week, beginning May 6, National Nurses Day, and continuing through May 12, the birthday of Florence Nightingale, the founder of modern nursing.

During this special week, Pemi-Baker recognizes and thanks its dedicated team of Hospice and Home Health nurses for their unwavering compassion, skill, and commitment to caring for patients and families throughout central New Hampshire.Happy National Nursing Week, Pemi-Baker Hospice & Home Health, Plymouth, NH

“Our nurses are at the heart of everything we do,” said Anna Swanson, Business Development Liaison at Pemi-Baker Hospice & Home Health. “They bring not only clinical expertise, but also comfort, dignity, and emotional support into the homes of those we serve every single day.”

Hospice and Home Health nurses play a unique and vital role in patient care:

  • Home Health nurses provide short-term, skilled medical care to patients recovering from illness, injury, or surgery. They help manage medications, monitor health conditions, educate patients and caregivers, and support individuals in regaining independence—all from the comfort of home.
  • Hospice nurses specialize in end-of-life care, focusing on comfort, pain management, and quality of life. They provide compassionate support not only to patients but also to their families, addressing physical, emotional, and spiritual needs during a deeply personal time.

By delivering care in patients’ homes, these nurses build meaningful relationships and ensure that individuals can receive high-quality, personalized care in a familiar and comfortable environment.

As the need for home-based care continues to grow, Pemi-Baker Hospice & Home Health is actively seeking compassionate healthcare professionals to join its team. Opportunities are available for Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Licensed Nursing Assistants (LNAs) who are dedicated to making a difference in the lives of others.

“Working in home health or hospice is incredibly rewarding,” added Swanson. “Our nurses have the opportunity to truly connect with patients and families and provide care that goes beyond medicine.”

Pemi-Baker Hospice & Home Health invites the community to join them in celebrating and thanking nurses during National Nurses Week. Their dedication and compassion make a lasting impact every day.

March is National Social Worker Appreciation Month

February 25, 2026

March marks National Social Worker Appreciation Month, and Pemi-Baker Hospice & Home Health is proud to recognize and celebrate the extraordinary medical social workers who play a vital role in caring for patients, families, and caregivers throughout some of life’s most challenging moments. Social Worker month Pemi-Baker Hospice & Home Health, Plymouth NH

Medical social workers bring compassion, expertise, and steady guidance to individuals facing serious and complex illnesses. Their work extends far beyond coordination of care — they provide emotional support, help families navigate difficult decisions, connect people with essential community resources, and ensure that patients’ voices and wishes remain at the center of every care plan.

For individuals living with serious conditions such as cancer, heart disease, Alzheimer’s, COPD, kidney disease, or recovering from stroke, the journey can feel overwhelming. Medical social workers help ease that burden by addressing not only physical needs, but also emotional, psychological, and practical concerns. They assist with advance care planning, offer counseling and coping strategies, support family communication, and help coordinate services that allow patients to remain comfortable and supported wherever they call home.

In hospice and serious illness care, their role becomes especially meaningful. Medical social workers provide guidance through complex healthcare systems, help families understand what to expect, and offer compassionate presence during times of uncertainty and grief. Their ability to listen deeply, advocate effectively, and respond with empathy makes an immeasurable difference in the lives of those they serve.

“Medical social workers are essential members of our care team,” said Aubrey Engle, RN, Executive Director of Pemi-Baker Hospice & Home Health. “They bring heart, knowledge, and advocacy together in a way that truly supports the whole person and the entire family. This month, we are honored to celebrate their dedication and the profound impact they make every day.”

As National Social Worker Appreciation Month is observed across the country, Pemi-Baker Hospice & Home Health encourages the community to recognize the critical role social workers play in supporting health, dignity, and quality of life — especially for those facing serious illness.

Pemi-Baker Hospice & Home Health is also currently seeking additional medical social workers to join our dedicated care team. Professionals who are passionate about providing compassionate, patient-centered support and making a meaningful difference in the lives of individuals and families are encouraged to learn more about career opportunities.

To learn more about social work services, hospice care, or employment opportunities, please contact Pemi-Baker Hospice & Home Health at 603-536-2232.

Hospice Care Offers More Help and Hope Than Many Realize

December 10, 2025

Facing a terminal diagnosis is one of the hardest moments for patients and the people who love them. Pemi-Baker Hospice & Home Health wants families to know that hospice care can bring comfort, guidance, and a stronger focus on quality of life during this time. You matter, Hospice care from Pemi-Baker Hospice & Home Health

Many people believe that starting hospice means death is near, but that is not always the case. Some patients even improve enough to graduate from hospice care. Pemi-Baker hopes to clear up common misunderstandings and help families see how much support hospice can offer.

To receive hospice services, a patient must have a terminal diagnosis with an expected life expectancy of six months or less. This is an estimate, not a deadline. Pemi-Baker encourages families to call as soon as they receive a diagnosis so a hospice consult can be done in the home. This gives the entire family a chance to ask questions, learn about available services, and find out if the patient qualifies. Hospice is covered under Medicare A benefits, which means the hospice provider pays for care related to the terminal diagnosis. The hospice care team at Pemi-Baker Hospice & Home Health, Plymouth, NH

Hospice care includes a wide range of services. These may involve skilled nursing visits, medical equipment, supplies, medications, social worker support, chaplain services, home health aides, bereavement support for families, and respite care. All services can be provided in the patient’s home or in a facility.

For patients living in a facility, hospice adds an extra layer of support on top of the care already being provided. This helps ensure the patient’s physical, emotional, and spiritual needs are met in a well-coordinated and personal way.

“Hospice is not about giving up. It is about helping people live their remaining time with as much comfort and dignity as possible, while also supporting the families who walk this path with them,” said Aubrey Engle, RN, Executive Director of Pemi-Baker Hospice & Home Health.

Pemi-Baker Hospice & Home Health hopes more families will explore hospice care as a positive and supportive option during a difficult stage of life.

Don’t wait to ask about how our compassionate staff can help your loved ones with the Gift of Hospice Care. To qualify for hospice care patients, families and providers can refer to the program if the patient’s condition is life limiting and the patient is seeking symptom management support at home or in a nursing home/hospital. Our focus is on providing comfort to patients and support to families, with our team available 24/7 to address any concerns. The call is free, and our professionals are ready to assist you! Hospice Care Is a Gift: Choosing Comfort, Dignity, and Quality of Life

June 14, 2025

Hospice care is a gift: Choosing comfort, dignity and quality of life with Pemi-Baker Hospice & Home Health.

Hospice care is often misunderstood. Too many families see it as a last resort—something to turn to only when there’s “nothing else to do.” But at Pemi-Baker Hospice & Home Health, we see it differently. Hospice is not about giving up. It’s about choosing how to live.

Hospice care is a gift—a chance to focus on what truly matters when time is short. It’s about comfort, dignity, and presence. It’s about saying yes to meaningful moments and no to unnecessary suffering.

“Everyday matters,” says the Pemi-Baker team. “Our mission is to help patients live fully and comfortably, on their own terms, surrounded by people who care.”

Pemi-Baker Hospice & Home Health offers expert medical support, pain management, emotional and spiritual care, and 24/7 access to a compassionate team who truly listens. The focus is on making each day the best it can be—at home, where patients feel most at peace.

Family members often describe a profound sense of relief once hospice begins. They no longer feel alone. Questions are answered. Symptoms are managed. And every detail—from medications to emotional support—is handled with deep respect and compassion. Pemi-Baker Hospice & Home Health team

Hospice is not just for the final days or hours. In fact, reaching out early allows patients and their families to get the most benefit from services. Choosing hospice early gives everyone more time to plan, connect, and find comfort in each other.

If you think you or someone you love might be ready for hospice, don’t wait. A simple phone call can make all the difference. The Pemi-Baker team is ready to guide you through your options, no pressure, just support.

“People often tell us, ‘I wish we’d called sooner,’” the team adds. “Hospice can bring calm, clarity, and connection during an incredibly difficult time.”

June is National Men’s Health Month

June 1, 2025

Pemi-Baker Hospice & Home Health Shines a Light on Men’s Health and Aging in Place During National Men’s Health Month.

June is National Men’s Health Month, and Pemi-Baker Hospice & Home Health is spotlighting the importance of healthy aging for senior men. As we age, maintaining strength and independence becomes critical to living well at home. One often overlooked but vital issue is Sarcopenia—the gradual loss of muscle mass that can occur with age or illness.

Sarcopenia affects nearly one in three adults over 60 and can significantly impact balance, mobility, and the ability to live independently. Without intervention, it increases the risk of falls, fractures, and a general decline in health and quality of life. For men hoping to age in place safely, addressing sarcopenia is essential.

What is Sarcopenia?
Sarcopenia is an age-related condition that leads to the progressive weakening and shrinking of muscles. It typically begins around age 50 and accelerates with inactivity, poor nutrition, and chronic illness. Symptoms include reduced strength, slower walking speed, and difficulty with everyday tasks like climbing stairs or rising from a chair.

What Can Men Do About It?
The good news: sarcopenia is not inevitable. Lifestyle changes can slow, stop, or even reverse muscle loss.

Here are some key strategies men can start at home today:

  • Strength Training: Simple resistance exercises—such as squats using a chair, wall push-ups, and lifting light hand weights—can rebuild muscle. Aim for two to three sessions per week.
  • Protein-Rich Diet: Consuming enough protein is essential. Incorporate lean meats, eggs, dairy, legumes, and nuts into meals and snacks.
  • Vitamin D: Seniors should get enough sunlight or consider supplements to maintain healthy muscles and bones.
  • Stay Active Daily: Walking, stretching, and gentle yoga can maintain mobility and prevent stiffness.

Pemi-Baker offers support for men and families navigating aging in place. Our physical and occupational therapy services, wellness programs, and at-home health assessments ensure that older adults stay strong, safe, and independent for as long as possible.

“This month, we encourage all men—especially those over 60—to take action,” said Ernest Roy, DPT, Quality Director. “Debilitating muscle loss does not have to be part of getting older. It’s a challenge we can fight—and win—with the right tools.”

To learn more about preventing sarcopenia and how Pemi-Baker can help men age in place with confidence, visit www.pbhha.org or call (603) 536-2232.

Hospice care with Pemi-Baker Hospice & Home Health, Plymouth, NH

“I would give your hospice team 10 stars if I could!”

They were truly amazing. We called on a Sunday, and they sent someone out that very afternoon to begin care and stayed by our side as Mom's health declined. We are especially grateful for Alexa, who went above and beyond to attend to our mom's needs and support us as caregivers. She was exceptionally knowledgeable, compassionate and simply an incredible human to be around. We couldn't have done this without her and will be forever grateful for her care. We also want to acknowledge Gretchen and Dr. Saunders, who guided us through some very challenging moments and took the time to truly listen, support us and answer all of our questions. I cant' say enough about our experience with hospice. We are deeply grateful for their care, compassion and support during this time.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
