September is Sepsis Awareness Month

August 30, 2024

Pemi-Baker Hospice & Home Health Urges Awareness During Sepsis Awareness Month.

September is Sepsis Awareness Month, a critical time to raise awareness about a condition that claims the lives of over 350,000 adults in the U.S. every year—more than opioid overdoses, breast cancer, and prostate cancer combined. Pemi-Baker Hospice & Home Health is committed to educating our community on the dangers of sepsis and the importance of early detection and prevention.

Sepsis is the body’s extreme response to an infection and is a life-threatening medical emergency. If not treated promptly, sepsis can lead to tissue damage, organ failure, and death. It is the number one cause of hospital readmissions in the U.S., costing the healthcare system more than $3.5 billion annually.

To help identify sepsis early, it’s essential to know the signs and symptoms using the acronym TIME: Sepsis Awareness Month Pemi-Baker Hospice & Home Health and Sepsis Alliance

  • Temperature: Higher or lower than normal.
  • Infection: Signs and symptoms of an infection.
  • Mental Decline: Confusion, sleepiness, difficult to rouse.
  • Extremely Ill: Severe pain or discomfort, shortness of breath.

Home Health Care Reduces the Risk of Sepsis

Early intervention is key to preventing sepsis from escalating. That’s why having our skilled home health nurses and aides in your home right after surgery can significantly reduce your risk of developing sepsis. At Pemi-Baker Hospice & Home Health, we provide personalized care designed to promote healing and prevent complications, ensuring our patients have the best possible outcomes.

Our commitment to excellence in patient care has been recognized nationally. Pemi-Baker Hospice & Home Health is proud to announce that we have been awarded the 2024 IPRO Quality Award for our outstanding achievements in reducing emergency department visits and improving transitions of care for our patients. This prestigious award underscores our dedication to enhancing the quality of care we provide to our community.

As we observe Sepsis Awareness Month, we encourage everyone to educate themselves about the dangers of sepsis and take proactive steps to protect themselves and their loved ones. The Journey Through Loss & Grief-New Support Group Starting!

August 26, 2024

Pemi-Baker Hospice & Home Health Starting New Grief Support Group Session: ‘The Journey Through Loss and Grief’

Throughout the ages of humankind, storytellers have spun tales which have fascinated the minds and imaginations of folks young and old as ways to help them discover their own inner strengths to confront the difficulties of life as we come to experience them. Probably no emotional journey is more challenging and difficult than that of loss and the feelings of grief that accompany it.

We do not have to wait long for such experiences. The boy next door, out of innocent curiosity drops a rock on a plastic rattle belonging to the little girl upstairs to see what makes its noise. The girl has lost her toy. The boy gets punished. Years move on. There are fun things that happen, but then the boy doesn’t make the softball team, or the girl doesn’t get the part in the play. Life moves on. She wins a prize in field hockey. Pets die. Boys or girls we develop crushes on don’t feel the same affection for us. Then we leave home to find our own paths in life. And on and on and on and on.

In between the good things, there are other things that hurt us, and we make our way on our journey, most of the time probably not knowing that we’re learning how to handle our losses and composing our own personal story. And then someone truly special we love is lost to us- by natural death, an accident, or by a violent crime, and our heart and soul hurt terribly and somehow, we need to move through all of that hurt if we are to continue to live and face a big challenge we’ve never had to face before. It helps to think about what we may have learned along the way so far and if that can help us now. Life is no fairy tale. But each of us has an interesting story.Guy Tillson, Hospice Chaplain, Pemi-Baker Hospice & Home Health, Plymouth, NH

The next six-week session, grief and bereavement group “The Journey Through Loss and Grief” offered by Pemi-Baker Hospice and Home Health will begin on Tuesday, September 10, 2024, at 10:30am in the upstairs art room (accessible by elevator) at the Plymouth Regional Senior Center. Please arrive before 10:15am to get a parking slip for the dashboard of your car from the receptionist at the front desk. There is no cost for participation. We are experiencing increased demand for participation. Please contact Guy Tillson, Hospice Chaplain, via email at gtillson@pbhha.org or by phone at 603 536-2232 to pre-register.

The Gift of Hospice Care

July 31, 2024

Pemi-Baker Hospice & Home Health is proud to announce the Gift of Hospice Care, a heartfelt initiative aimed at educating the community about the profound benefits of opting for hospice care early as loved ones age. This initiative underscores the compassionate, comprehensive support available to patients and their families during life’s final stages.

  • Opting for hospice care is a compassionate choice, not a negative one.
  • Selecting hospice care is a loving decision, not something to fear.
  • Embracing hospice care is a blessing, not a burden.

Hospice care provides a holistic approach to end-of-life care, focusing on the physical, emotional, and spiritual needs of patients. By choosing hospice care early, patients receive personalized medical care designed to manage pain and symptoms, allowing them to live their remaining days with dignity and comfort.

Families also experience significant benefits when hospice care is initiated early. They gain access to a dedicated team of healthcare professionals, including nurses, social workers, chaplains and volunteers who provide guidance and emotional support. This network of support helps families navigate the complexities of end-of-life care, offering respite and ensuring that they are not alone during this challenging time.Dr. Kristi Saunders, Hospice Medical Director, Pemi-Baker Hospice & Home Health, Plymouth, NH

Additionally, early hospice care can lead to better quality of life for both patients and their loved ones. It enables more time to create meaningful memories, facilitates important conversations, and provides peace of mind knowing that compassionate, expert care is readily available day or night.

“Our mission is to ensure that patients and families receive the best possible care and support during one of life’s most natural transitions,” said Dr. Kristi Saunders, MD, Hospice Medical Director at Pemi-Baker Hospice & Home Health. “By choosing hospice care early, families can focus on spending quality time together while we take care of the rest.”

Don’t wait to ask about how our compassionate staff can help your loved ones with the Gift of Hospice Care. Raising Awareness About Diabetes and Home Health Care Solutions

July 30, 2024

Pemi-Baker Hospice & Home Health: Raising Awareness About Diabetes and Home Health Care Solutions.

Pemi-Baker Hospice & Home Health is committed to raising awareness about diabetes and providing comprehensive care to those affected by this chronic condition. With the number of diabetes cases rising, understanding the disease and knowing the available support options is more important than ever. Pemi-Baker Hospice & Home Health Diabetes patient Testing blood sugar

Diabetes is a serious health condition that affects how your body turns food into energy. Most of the food you eat is broken down into sugar (glucose) and released into your bloodstream. When your blood sugar goes up, it signals your pancreas to release insulin. Insulin acts like a key to let the blood sugar into your body’s cells for use as energy. With diabetes, your body doesn’t make enough insulin or can’t use it as well as it should. When there isn’t enough insulin or cells stop responding to insulin, too much blood sugar stays in your bloodstream, which over time can cause serious health problems, such as heart disease, vision loss, and kidney disease.

For those who qualify, home health care can be a crucial resource in managing diabetes. Our skilled healthcare professionals at Pemi-Baker Hospice & Home Health provide personalized care plans to help patients manage their condition effectively at home. Our services include:

  • Education and Training: Our nurses educate patients and their families about diabetes management, including medication administration, blood sugar monitoring, and lifestyle changes.
  • Medication Management: Ensuring patients take their medications correctly and on schedule.
  • Nutritional Guidance: Our nurses offer guidance and advice to help manage diabetes through diet.
  • Regular Monitoring: Frequent check-ins to monitor blood sugar levels and overall health, allowing for prompt adjustments to care plans as needed.
  • Support and Counseling: Emotional and psychological support to help patients and their families cope with the challenges of living with diabetes.

For patients who qualify for hospice care, our team provides compassionate end-of-life care that focuses on comfort and quality of life. Our hospice services include pain and symptom management, emotional and spiritual support, and assistance with advanced care planning.

Don’t wait to ask about how our compassionate staff can help you or your loved ones living with diabetes. To qualify for home health care, a person must have a physician’s order and demonstrate a need for skilled care that can be safely provided at home. Pemi-Baker Hospice & Home Health Highlights the Benefits of Home Health Care and Hospice Care for COPD Patients

Chronic Obstructive Pulmonary Disease (COPD) is a serious condition characterized by damaged airways in the lungs, leading to symptoms such as shortness of breath, low oxygen levels in the blood, persistent coughing, pain, weight loss, and an increased risk of lung infections. The emotional toll of COPD is significant, often resulting in depression, anxiety, insomnia, and social isolation.

At Pemi-Baker Hospice & Home Health, we understand the complexities of managing COPD and are committed to providing comprehensive support through our home health care, palliative care, and hospice care services. Nurse helping COPD patient, Pemi-Baker Hospice & HOme Health, Plymouth, NH

Home Health Care: Enhancing Quality of Life at Home

Home health care offers personalized medical care in the comfort of a patient’s home. For COPD patients, this means access to skilled nursing, physical therapy, and occupational therapy tailored to their specific needs. Our team works closely with pulmonologists and other healthcare providers to ensure seamless care coordination. To qualify for home health care, a person must have a physician’s order and demonstrate a need for skilled care that can be safely provided at home. The benefits include:

  • Improved Symptom Management: Regular monitoring and adjustment of treatment plans help manage symptoms such as pain, shortness of breath and fatigue, more effectively.
  • Reduced Hospital Visits: Early intervention and continuous care reduce the likelihood of emergency hospital visits.
  • Patient and Family Education: Teaching patients and their families about COPD management empowers them to take control of their health. Counseling and support are offered to cope with depression, anxiety, and other emotional challenges.

Hospice Care: Compassionate End-of-Life Support

For COPD patients in the advanced stages of the disease, hospice care offers compassionate support aimed at enhancing the quality of life. Our hospice services provide:

  • Pain and Symptom Management: Ensuring patients remain comfortable and pain-free.
  • Emotional and Spiritual Support: Offering counseling and support for patients and their families during this challenging time.
  • 24/7 Availability: Providing around-the-clock care and support to address any urgent needs.

While pulmonologists play a crucial role in the medical treatment of COPD, the management of symptoms and stress associated with the disease requires a comprehensive approach. Pemi-Baker Hospice & Home Health Expresses Gratitude for Generous Donation from Corey Goodwin Tribute Motorcycle Ride

Pemi-Baker Hospice & Home Health extends heartfelt thanks to Dave Hooper, Jeanne Goodwin, friends, and family for their recent generous donation. Funds were raised through the Corey Goodwin Tribute motorcycle ride, organized by Dave ‘Hoop’ Hooper. This event marks Dave’s 24th organized motorcycle poker run, traditionally held on the Thursday of Laconia Bike Week each year.

Every year, riders, friends, and family participate in the 130-150 mile journeys, supporting various local charities. This year’s ride was particularly poignant, honoring ‘Captain’ Corey Goodwin, a long-time rider who sadly passed away in January after battling Pancreatic Cancer. Corey’s widow, Jeanne Goodwin, a consistent participant in the event, took part in the ride and requested that the funds raised be donated to Pemi-Baker Hospice & Home Health, who provided compassionate care to Corey until the end.

Pemi-Baker Hospice & Home Health is extremely grateful for the support and generosity demonstrated by all participants. This contribution will significantly aid in continuing our non-profit mission, ensuring every family receives the compassion, education, and assistance they need, regardless of their ability to pay.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.

“Everyone is so understanding supportive and kind.”

From the director to the nurses to the health care workers.  So cheerful, helpful and accommodating.  This is our second time receiving assistance from Pemi-Baker and we are very lucky to have found this amazing group!  Thank you for your excellent care and compassion.

“Pemi-Baker Hospice services provided kind, professional, competent services every step of the way-and even vaccinated us all for Covid/19!””

Our primary nurse, Fred, was outstanding, and others who filled in (Cathy, Penny, Colleen) were wonderful and a credit to your organization. Fred, you were amazing, not only for mom but for my husband and I as well. I will forever be grateful for your support, wisdom, humor and kindness, all along the end of life journey for mom. I am so deeply thankful that circumstances conspired such that mom was able to die at home, and your role made that time as seamless as it could be made.

Your courage during Covid-19 does not go unnoticed

Hello. We just wanted to take a moment and let you know how much we truly appreciate all that you do for us in these trying times. You're commitment to the health of our community is a special thing, and we know that you are working under stressful conditions due to the Covid 19 crisis. We realize that in order for you to treat those who may be ill, you are putting you're own lives and even you're own families lives in danger, and that is truly a courageous and honorable thing to do. We want you to know that it does not go unnoticed. Thank you all for all that you do!
