Living with heart failure can be an uphill battle, often characterized by breathlessness, anxiety, fatigue, and discomfort due to swelling in the extremities. These symptoms can significantly limit one’s ability to engage in daily activities and diminish the enjoyment of life’s pleasures. Recognizing these challenges, our palliative care team steps in to provide comprehensive support and alleviate distress.
Palliative care, pronounced “pal-lee-uh-tiv,” is a specialized form of medical care designed to address the symptoms and stresses associated with serious illnesses such as heart failure. Our multidisciplinary team of doctors, nurses, and specialists collaborates closely with patients’ existing healthcare providers to offer an additional layer of support. Regardless of age or stage of illness, palliative care focuses on improving the quality of life for both patients and their families.
Heart failure presents a range of unpredictable symptoms, from fluid retention and breathing difficulties to pain and anxiety. Our expert palliative care team is adept at managing these symptoms and helping patients navigate the challenges they face on a day-to-day basis. Whether it’s providing strategies to improve breathing or offering techniques to reduce anxiety, our team is dedicated to empowering patients with the tools they need to cope effectively.
Beyond symptom management, our palliative care professionals serve as guides, assisting patients and their loved ones in navigating the complexities of the healthcare system. From facilitating discussions about treatment options to ensuring effective communication among healthcare providers, we strive to help patients make informed decisions about their care.
As we observe National Heart Health Month, Pemi-Baker Hospice & Home Health reaffirms its commitment to providing compassionate palliative care for individuals living with heart failure. Through our tailored services, we aim to alleviate suffering, enhance comfort, and promote dignity for patients and their families throughout their journey.

Pemi-Baker Hospice & Home Health Addresses the Challenges of Caregiving and Extends Support Services.

Pemi-Baker Hospice & Home Health recognizes the importance of supporting caregivers in their journey and is committed to providing resources and assistance to make the caregiving experience more manageable.Caregiving support groups Pemi-Baker Hospice & Home Health, Plymouth, NH

To help caregivers cope with the demands of their role, Pemi-Baker encourages individuals to reach out for support. Whether it’s talking to a trusted family member or friend or seeking counseling from a mental health professional, having a support system is crucial. Pemi-Baker recognizes the need for in-person support and invites caregivers to join their support group, held every first Wednesday of the month from 10:00 AM to 11:30 AM. This group offers a space for caregivers to connect, share experiences, and gain valuable insights from others who understand the challenges they face.

Taking breaks and prioritizing self-care are essential aspects of caregiving. Pemi-Baker suggests that caregivers consider asking another family member or friend to step in, hiring an aide for a few hours a week, or enrolling the older person in an adult day care program to ensure they have the necessary support.

In a significant expansion of services, Pemi-Baker is pleased to announce the introduction of volunteer services for home health patients and their families. While volunteer services were previously available only for patients on hospice care, this extension aims to provide additional assistance to individuals receiving home health services. Holiday Blues Grief & Loss Support Group November 21st, 2023

November 1, 2023

Pemi-Baker Hospice & Home Health Starting Free Holiday Blues Grief & Loss Support Group November 21, 2023

Here in the United States, it seems like “the holidays” are now starting at Halloween. But as Thanksgiving approaches, the black cats, witches, ghosts, gravestones, and skeletons have to exit to make room for the turkeys, even though the pumpkins, gourds, and autumn leaves remain. But look! Behind them are the tree decorations, wreaths, ornaments, lights, snowmen, and wrapping paper!

The colors, sparkle, and glitz of the holidays, however, can leave persons unfazed who have endured losses of any kind. Someone will be missing from the Thanksgiving table this year. ‘Remember how Dad used to lose it and swear when hanging the lights outside?’ ‘We just moved here from Oregon in August and we don’t really know anyone yet.’ ‘Who’s going to make the potato latkes for Hanukkah now that Bubbe is gone?’ ‘I got laid off three weeks ago and haven’t been able to find work.’ ‘We had to put Princess down- she was so sick.’ ‘What’s New Year’s going to be like without Uncle Jack to snowmobile with or to watch the games on TV?’ The season of celebration and merriment can be an emotionally difficult time for those unsettled and disturbed by bereavement.

As in previous years, Pemi-Baker Hospice & Home Health will be offering its “HOLIDAY BLUES Grief and Loss Support Group” for those in need of processing various losses and in need of support in the company of others.

The group will begin on Tuesday, November 21, 2023 and will go for six sessions, concluding on Tuesday, December 26, 2023. The group meets at 10:30am in the upstairs art room (accessible by elevator) at the Plymouth Senior Center. The group process will include such handouts as “A Holiday Grievers Bill of Rights” to stimulate conversation and sharing as well as ideas for simple rituals that can be taken home. We strive toward providing an emotionally safe space where persons can find understanding and strength.

Persons who wish to participate must pre-register by contacting Guy Tillson, Hospice Chaplain and Bereavement Coordinator by email at gtillson@pbhha.org or by phone at (603) 536-2232.  Please contact him BEFORE Wednesday, November 15, 2023, so we have an idea of how many to prepare for.

Pemi-Baker Celebrates Fall Prevention Month in September

September 6, 2023

Pemi-Baker Hospice & Home Health is proud to announce its participation in Fall Prevention Month this September. Falls are the leading cause of injuries among older adults, and Pemi-Baker is committed to raising awareness and promoting safety measures to prevent these accidents within the home.

Each year, millions of older adults suffer from fall-related injuries, leading to hospitalizations, reduced quality of life, and increased healthcare costs. Fall Prevention Month serves as a vital opportunity to educate the community about the importance of fall prevention and offer guidance on creating safer home environments for older adults.

According to the Centers for Disease Control and Prevention (CDC), falls are responsible for numerous injuries, including fractures, head injuries, and even fatalities. The physical, emotional, and financial toll of falls is significant, and Pemi-Baker Hospice & Home Health is dedicated to making a positive impact by sharing valuable information and resources on fall prevention.Tai Chi Class

Key Ways to Prevent Falls in the Home:

  • Home Safety Assessment: One of the first steps in fall prevention is conducting a thorough home safety assessment. Identifying and addressing potential hazards like loose rugs, cluttered walkways, and poor lighting can significantly reduce the risk of falls.
  • Medication Management: Many medications can cause dizziness or unsteadiness, increasing the likelihood of falls. Regular medication reviews with healthcare providers can help adjust doses or find safer alternatives.
  • Exercise and Balance Training: Engaging in regular exercise, especially strength and balance training, can improve stability and coordination, reducing the risk of falls. Activities like tai chi and yoga are particularly beneficial.
  • Vision Care: Regular eye check-ups are crucial as poor vision can lead to falls. Ensure that eyeglass prescriptions are up to date, and adequate lighting is available in all areas of the home.
  • Assistive Devices: The use of assistive devices like grab bars, handrails, and non-slip mats in bathrooms and other high-risk areas can significantly enhance safety.
  • Footwear: Encourage the use of proper footwear with non-slip soles and good arch support, both indoors and outdoors.
  • Stay Hydrated and Nourished: Dehydration and poor nutrition can lead to weakness and unsteadiness. Maintaining a balanced diet and staying hydrated are essential for overall health and fall prevention.
  • Home Health Services: Pemi-Baker Hospice & Home Health offers home health services, including skilled nursing and physical therapy, to help older adults maintain their independence and reduce fall risk.

“We are dedicated to the well-being of our community, especially its older members,” said Danielle Paquette-Horne, RN, Executive Director of Pemi-Baker Hospice & Home Health. “We hope to empower individuals with the knowledge and tools they need to stay safe in their homes and enjoy a higher quality of life.”

Pemi-Baker Hospice & Home Health encourages everyone to join in their efforts to prevent falls and promote safety within the home. For more information and to access free resources, or contact our team at 603-536-2232.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.

To the Pemi-Baker Hospice Team--Words really can't express my feelings-

Your help during one of the most difficult times of my life is SO appreciated.  I'm so grateful that because of you, Doug was able to get his wish not to spend the last of his days in a hospital, but at home with those of us who loved him.

And to be constantly reassured that if I needed anything you would be there-oh my gosh-ANYTIME!  Well, you people need to know how much you are appreciated.  You give of yourselves day & night without end!  God Bless you and thank you!

“My home physical therapy was fantastic.”

Pemi-Baker provided very good home therapy. All their therapists were pleasant, helpful in offering suggestions for better functioning after surgery, and practiced good health protocols. They were also very prompt in showing up for scheduled appointments.

“Your generosity with your time and your loving care is appreciated more than you know.”

Thank you to everyone who played a role in taking care of my mother and giving her the freedom of remaining home while suffering from dementia. Your generosity with your time and your loving care is appreciated more than you know. A special thanks goes to Macayla and Colleen, who visited her on a regular basis, and to Lauren who was helpful in connecting us with state services.

“Everyone is so understanding supportive and kind.”

From the director to the nurses to the health care workers.  So cheerful, helpful and accommodating.  This is our second time receiving assistance from Pemi-Baker and we are very lucky to have found this amazing group!  Thank you for your excellent care and compassion.

“Pemi-Baker Hospice services provided kind, professional, competent services every step of the way-and even vaccinated us all for Covid/19!””

Our primary nurse, Fred, was outstanding, and others who filled in (Cathy, Penny, Colleen) were wonderful and a credit to your organization. Fred, you were amazing, not only for mom but for my husband and I as well. I will forever be grateful for your support, wisdom, humor and kindness, all along the end of life journey for mom. I am so deeply thankful that circumstances conspired such that mom was able to die at home, and your role made that time as seamless as it could be made.

Your courage during Covid-19 does not go unnoticed

Hello. We just wanted to take a moment and let you know how much we truly appreciate all that you do for us in these trying times. You're commitment to the health of our community is a special thing, and we know that you are working under stressful conditions due to the Covid 19 crisis. We realize that in order for you to treat those who may be ill, you are putting you're own lives and even you're own families lives in danger, and that is truly a courageous and honorable thing to do. We want you to know that it does not go unnoticed. Thank you all for all that you do!
