How to Plan For Recovery at Home After Surgery

In this blog, Pemi-Baker Hospice & Home Health brings to light the services you can receive after surgery and how to be prepared before your surgery. There are a lot of things to think about when preparing for major surgery. What are the risks? Will you be in pain? Have you thought about what help you’ll need to recover? Will you be able to stay at home while you heal? Although every surgical recovery is different, the goals of home-based clinical care after surgery include patient and family involvement, shared decision-making, and flexibility around changes in plans to meet your needs.

Having home care services from Pemi-Baker can help you recover more quickly, keep you safe at home, and just as importantly, help you avoid another hospital stay.

Planning ahead for care

Although it’s not always possible to schedule your surgery in advance, you should understand beforehand how long your recovery may take, what you will and will not be able to do, and what help you will need while you are home.

Speaking to your doctor or your hospital discharge planner about home health care services prior to surgery will help you prepare for when you go home again. If your surgery is the result of an emergency situation, a family member or other caregiver could arrange for services on your behalf.

Additionally, since outpatient (day) surgery has become a much more common option, many people find themselves recovering from surgery at home the same day as their procedure. Some are unprepared to effectively manage their pain and discomfort, making it difficult for them to perform even routine tasks or basic personal care. They may be weak and tired. How home health care helps your recovery

Home health care is personalized to fit your needs, based on your physician’s instructions and your own personal preferences. Depending on the complexity of the surgery, you may receive a single type of care or a combination of services. Nurses can provide services including wound care and medication management, while home health aides can provide assistance with personal care including bathing, dressing, and light housekeeping. Physical and occupational therapists, speech language pathologists, and social workers also play an important role in the recovery process. With orders from your doctor and input from you about your needs, the Pemi-Baker team will design a plan of care to reach your goals and help you recover.

What you can do ahead of the surgery

You can make your recovery at home go a lot smoother if you take a little time to plan ahead. Ask your doctor what you won’t be able to do at first while you’re healing up, and get help from friends and family to prepare your living space. You need to be careful about falling after an operation. “People get dizzy because they haven’t eaten for a few days and from being in bed,” says Jenn Stewart, RN, Clinical Supervisor. “You may need to get a walker or crutches to cut down your risk of a fall. Other tips to avoid stumbling would be using night lights in hallways, sleeping in a bedroom near a bathroom, wearing flat shoes or slippers and reducing the clutter in your home” said Stewart.

  • Do I need to make changes to my house? This depends on what kind of operation you have. If it’s a complicated one, like a joint replacement or stomach surgery, you may need to tweak a few things in the bathroom or bedroom for easier access.
  • Stairs: If you can’t climb up and down them after your surgery, you might have to make some changes. For instance, if your bedroom is upstairs, you may need to sleep on a lower floor for a while. Ask your friends or family to move your bed before you go into the hospital, or check into renting a hospital bed if you need it.
  • Equipment: After some types of surgery, you’ll need to get special gear at home. Plan ahead if your doctor tells you that you need oxygen tanks, elevated toilets, shower seats, supplies to care for your cut, or other items. Get in touch with your insurance company to see if they’re part of your coverage.
  • Stock your pantry: Ask your doctor if you have to stick to a certain diet. Make sure you’re well supplied with the right food before you get back home. Since you may feel tired during your recovery, prepare some meals before your operation and put them in the freezer.

There are many things you and your family can do to prepare for your surgery and give you the best possible recovery. Make arrangements in advance if your surgeon suggests getting a nurse, physical therapist, or health aide. Make sure you schedule the first visit before you leave the hospital. How to Manage Fatigue When Living with a Serious Illness

When living with a serious illness, it’s important to know that feeling easily and constantly tired—or fatigued—is a common symptom. In fact, many people experience fatigue, no matter if they are living with cancer, COPD, kidney disease, or another serious illness. Fatigue may make you or your loved one feel forgetful and unable to stay focused or have energy to do everyday things. This may include activities like seeing friends, pursuing hobbies, or even pursuing treatment for the serious illness.

While fatigue is not always easy to recognize, learning about its symptoms is a good place to start. Since fatigue can impact quality of life, it is important to get it under control so that you or your loved one can start to feel better. If you aren’t sure where to start or need support along the way, palliative care from Pemi-Baker Hospice & Home Health can help.

What is Palliative Care?

Palliative care is specialized medical care for people living with a serious illness. This type of care focuses on providing relief from the symptoms and stress of the illness. The goal is to improve quality of life for both the patient and the family. Palliative care is provided by a specially trained team of doctors, nurses and other specialists who work together with a patient’s other doctors to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness, and it can be provided along with curative treatment.

How Can Palliative Care Help?

If you notice that you or your loved one are constantly tired, unable to focus, withdrawn, or just not doing things you normally enjoy, let your doctor know. They will ask you questions and try to figure out what may be causing it.  This could mean anything from low blood count, pain, medications, anxiety and/or depression, and more. There are different ways to treat fatigue, depending on the cause. Either way, your doctor can refer you to palliative care, to help manage the fatigue and get back to doing what is most important to you.

During your first visit with the palliative care team, let your doctor, nurse, or social worker know that you’ve been feeling fatigued. They will continue to evaluate your fatigue by asking about sleep disturbances, pain, nutrition, emotional distress, and more. And they will work with you to come up with ways to start feeling better.

This may mean following a regular sleep schedule, avoiding caffeine, choosing relaxing activities before bed, and more. But it all depends on the root cause, and you will figure this out together.

November is Home Care & Hospice Month

More and more, people prefer to be cared for in the comfort and dignity of their own homes. Pemi-Baker Hospice & Home Health provides in-home medical services that are cost-effective and person-centered. November is a chance to recognize the dedication and innovation of providers of health care at home. It is an opportunity to acknowledge recipients of in-home care services, and celebrate that they are able to remain in the safety and comfort of their own homes while receiving quality care.

Home health, hospice & palliative care providers have always been important to vulnerable citizens in our communities. But now that importance is underscored by the risk of exposure to COVID-19 in public/facility settings. When given the option, patients want to receive care in their homes. We believe that desire is stronger today than it has ever been. Pemi-Baker Hospice & Home Health is proud to support 29 communities in central and northern NH.

Home Health

Home Health provides skilled services, including nursing care, physical, occupational and speech therapy. Pemi-Baker will send a licensed professional to assess home health needs and develop a complete care plan. These services are usually short-term, as in the case of a person recovering from surgery or other health issues.

What are the costs and what is covered?
Medicare will cover home health if it is ordered by a physician and the person is homebound and in need of skilled services. Services are usually covered for a 30-day “episode of care.” However, if an individual is enrolled in a Medicare HMO, known as a Medicare Advantage Plan, services are authorized on a per-visit basis. Long-term care insurance and Medigap plans may also cover some costs of a home health aide.

Hospice Care

Hospice care brings together medical care, pain management and emotional and spiritual support for patients at the end of life and their families. The mission of hospice staff and volunteers is to address the symptoms of a terminal illness with the intent of promoting comfort and dignity. They are experts at pain management.

Hospice will manage your loved one’s pain, help you understand what he or she is going through and help you and other family members cope with your emotions. Many of us are at a loss as to what we should do before and at the time of death. Hospice professionals help you get through a very difficult time and are there even after your loved one’s death.

What are the costs and what is covered?

Medicare and Medicaid provide a hospice benefit that covers almost all of the costs of caring for a dying person during his or her last six months of life. To qualify for the Medicare hospice benefit, a person must have:

  • Medicare Part A
  • A life expectancy of less than six months
  • An agreement in writing that he or she will not pursue any treatments to cure his or her illness

The Medicare hospice benefit covers skilled nursing services, physician visits, skilled therapy (i.e. physical, speech or occupational), medical social services, nutrition counseling and bereavement counseling, most of the cost of prescription drugs for symptom control and pain relief. Short-term inpatient respite care to relieve family members from care-giving is also covered.

Holiday Grief and Loss Support Groups

The merry and festive atmosphere of the upcoming fall and winter holidays can pose a challenge to those who have endured significant losses. The losses do not have to be the deaths of loved ones. They can include such things as a divorce or the breakup of a relationship, being laid off from a job, relocation from a long-familiar setting to a brand-new place where all seems strange, the death of a pet, or the loss of physical health and powers due to a medical condition.  In any case, one may not feel at all like celebrating even though it seems like everyone else does.

To address and explore this concern, Pemi-Baker Hospice & Home Health will be offering TWO support groups beginning on Tuesday, November 22, 2022. A morning group will meet in-person at 11:00am at the Plymouth Regional Senior Center. A late afternoon virtual meeting group will begin on the same date at 5:30pm. Those interested in this group will need to download the app for TEAMS on their computer.

The groups meet for SIX sessions (all Tuesdays) on November 22, November, 29, December 6, December 20, and December 27, 2022; and, January 3, 2023. Please note there is NO MEETING on Tuesday, December 13, 2022. The energy generated by the group discussion and sharing determines the length of the group session, though we will definitely conclude by 12:30pm for the morning group and by 7:00pm for the late afternoon group. Group sessions include a combination of educational presentations, shared discussion, and reflective activities.

There is no cost to participate in either group; however, you MUST pre-register to attend by contacting Guy Tillson, Hospice Chaplain and Bereavement Coordinator, by email gtillson@pbhha.org or by phone 603 536-2232.  The groups are a service offered to our larger community by Pemi-Baker Hospice & Home Health.

In-Home Physical Therapy Helping Breast Cancer Rehabilitation

October is National Physical Therapy month as well as Breast Cancer Awareness month and the physical therapists at Pemi-Baker Hospice & Home Health can assist patients with breast cancer to live a more functional life. Often there is a missing link once a person is diagnosed with breast cancer. A patient can go through the whole diagnosis, yet have many questions about how to return to their prior level of function or how to progress themselves into strength training or exercises after treatment. Pemi-Baker therapists will come to your home, answer your questions and help you on that path of recovery.

While medical treatment for breast cancer is saving more lives, the disease may leave patients with significant side effects from chemotherapy and/or radiation causing treatment induced fatigue.

If a person undergoes surgery, there can be significant scar tissue, which causes limitations of everyday movement.  After having a lumpectomy, mastectomy and/or breast reconstruction people may experience scar tissue restrictions, swelling, shoulder, neck or back pain. If a person is feeling tightness, pain, loss of range of motion or strength, or feels like their posture is being impaired, these can be signs identifying the need for physical therapy.

Pemi-Baker Hospice & Home Health Celebrates Gazebo Dedication

The sun shone brightly at Pemi-Baker Hospice & Home Health’s special gazebo dedication to the Pemi-Baker staff in memory of Frances Marie Fernandes, a former hospice patient. John Fernandes and his two daughters have been long supporters of the Pemi-Baker Hospice and Palliative care programs but they wanted to give back specifically to the staff, giving them a place to rest, reflect and rejuvenate. 

The ornate, covered gazebo rests on the lawn outside the offices at Boulder Point, overlooking the pond and mountains to the north of Pemi-Baker Valley. “Fran would want you to enjoy this gazebo because it is a link to the beauty of nature. Maybe on a pleasant day, sitting here and contemplating the beautiful scenery, you will feel something of the appreciation our family has for what you do. We hope you will continue to enjoy this place for many years to come,” said John Fernandes.

PBH&HH Score High in Hospice Satisfaction Survey Results

According to the most recent independent Strategic Healthcare Program (SHP) report comparing agencies of the Rural Health Care Network (RHCN), Pemi-Baker Hospice & Home Health (PBH&HH) shines bright in all categories and is at the top or tied for the top in over 20 categories of patient satisfaction for Hospice. We are touched and honored that our hospice families gave us such high scores with regard to the care their loved ones received.

The report measures many details about the agency’s interactions and care including communication with the family, getting timely help, treating patients with respect, emotional and spiritual support, help for pain and symptoms and training the family to care for the patient. Though PBH&HH scored high on all survey questions the following items scored a 100%-actual positive score:

  • “Communication with the family”-composed of separate questions, including-
  1. Things were explained in a way that was easy to understand,
  2. Hospice team listen carefully about any problems with care,
  3. Family kept informed about patient’s condition
  • “Treating the patient with respect”-composed of 2 separate items including-
  1. How often patient was treated with dignity and respect
  2. You felt the hospice team really cared about the patient
  • “Percentage of families who would rate the Hospice care as a 9 or 10 on a 0-10 scale”
  • “Would you recommend this Hospice to family or friends?”

It’s Make-A-Will Month

August is Make-A-Will Month. It’s the perfect moment to make your mark on all that you care about. On average it only takes 20 minutes to write or update your legal will. Still procrastinating? Read 10 great reasons to start today:

  1. Save your loved ones time, money and stress. Most estates go through probate to start the legal process for distributing assets upon your death. Without a will, this gets complicated because the court names an administrator. With a will, you choose who will handle your estate, making it easier and smoother for your loved ones.
  2. Determine who will manage your estate. Choose an executer who is capable and trustworthy to carry out your wishes.
  3. Decide who gets your assets and property. Name people or charitable organizations as beneficiaries for specific assets or any residual value of your estate. Did you know you can bequest a gift to Pemi-Baker Hospice & Home health? It’s as easy as adding a few simple sentences. Join their Dorothy Westberg Heritage Society, named after their first nurse who saw the vision in 1967. This society of individuals are including PBH&HH in their wills, IRA’s, Trusts, Life Insurance and Retirement Plans, helping Granite Stater’s in need…now and in the future.
  4. Choose who will care for your minor children. This is one of the most important reasons to have a will. Typically, a surviving parent will get sole legal custody of minor children. However, you can name a guardian who will be responsible for your children’s daily needs, including food, housing, health care, education, and clothing. Without a named guardian, a court will decide who will raise your kids.
  5. Provide a home or plan for your pets. Name a beneficiary or caretaker for your pets and set aside funds to provide for their care.
  6. Leave instructions for digital assets. An executer can be named to manage digital assets such as online accounts, social media, email and digital files.
  7. Lower the potential for family disputes. If you die without a will, your family has to guess what your final wishes were. Chances are, they may not agree. This ambiguity can create friction, and even fights, which sometimes last a lifetime. Making a will solves this problem by eliminating guesswork.

The peace of mind and relief you feel when Hospice comes in to help care for your loved one is almost instant. In fact, most people only wish that they had called sooner. Our wish is that more of our friends and neighbors in the community would know to turn to our professional caregivers at Pemi-Baker Hospice and Home Health in their time of need. And, it’s never too soon to call us and just ask questions! We can generally help sooner than you think.

Pemi-Baker Hospice & Home Health provides their care in your home or where you call home. You can’t ask for a better Hospice organization and that is why we are sharing the following information with you in the hopes that you can help your loved ones get the best care when in need.

  1. Hospice is not about giving up hope. It’s about embracing hope and living longer with exceptional care. Facing the physical and emotional challenges of a life-limiting illness is overwhelming. You owe it to yourself to seek every option. Calling hospice does not mean giving up hope – it means letting hope in, for help and support, when you need it most. Our care is designed to help patients live more fully and comfortably – with a team of caregivers who work to ease symptoms and enhance quality of life. It is choosing compassionate care and support so that you can be at home – wherever you call home.
  2. Patients and families who choose hospice care live longer and report feeling better, emotionally and physically, almost immediately. With improved quality of life, hospice patients actually live longer! A study published in the Journal of Pain and Symptom Management showed that patients who choose hospice care live an average of 29 days longer than patients who do not have hospice care.
  3. Remarkably, there is rarely any out-of-pocket expense for hospice care. You have already paid for hospice care. It is an entitlement your tax dollars earn you. Hospice care is most often paid for by Medicare, Medicaid, private insurance, or donations. Therefore, in addition to easing physical and emotional burdens, using hospice at end of life can ease financial burdens. The Medicare Hospice Benefit is available to all patients with the Medicare part A benefit.
  4. Hospice care is designed to support people for months. Far too many patients and families say—“We wish we had called sooner.” Hospice care is not simply for patients in the final days or weeks, but rather months. Patients can receive hospice care as soon as a physician predicts that life expectancy is six months or less. Anyone, including yourself, family or friends, can make a referral to hospice. In fact, we provide free in home consults to you and your family about your current or future options.
  5. Hospice care is provided in your home—wherever you call home. Our teams of caregivers come to you, making routine visits so that you can remain comfortable in your home with family, friends and familiar surroundings. This includes private homes, assisted living facilities and/or skilled nursing facilities. “I wish we had called sooner.” That’s what most families say once they have embraced the support of hospice. We understand that until you have had a first-hand experience with hospice care, you might not understand the true essence of hospice. The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
