Giving Tuesday, give to Pemi-Baker Hospice & Home Health, Plymouth, NH

As the holiday season approaches, Pemi-Baker Hospice & Home Health invites the community to embrace the spirit of giving by becoming sustaining donors on Giving Tuesday, celebrated nationally on November 28th. This year, Pemi-Baker encourages individuals to join their ‘Circle of Friends,’ a monthly giving club that supports under-insured neighbors and ensures the continuation of vital community services.Circle of Friends, Pemi-Baker Hospice & Home Health, Plymouth, NH

The ‘Circle of Friends’ initiative empowers individuals to make a lasting impact with a commitment of just $15 a month. By joining this monthly giving club, community members contribute to the sustainability of Pemi-Baker’s free community services, including Bereavement & Caregiver support groups, Ask a Pemi-Baker Nurse Senior Center days, and free Hospice volunteer training programs.

“Pemi-Baker is deeply rooted in its mission to provide compassionate care and support to individuals and families during some of life’s most challenging moments,” said Dr. Diane Arsenault, MD, Interim Executive Director at Pemi-Baker Hospice & Home Health. “This Giving Tuesday, we invite our community to join hands with us and become a part of our ‘Circle of Friends,’ making a meaningful difference in the lives of our neighbors.”

The $15 monthly donation goes a long way in sustaining the high-quality care and services that Pemi-Baker provides to the community. It ensures that those facing difficult times have access to the support and resources they need.

To join the ‘Circle of Friends,’ interested donors can conveniently sign up online HERE. The simple, secure process allows individuals to become sustaining donors with just a few clicks, enabling Pemi-Baker to continue its commitment to enhancing the well-being of the community.

As the season of giving unfolds, Pemi-Baker Hospice & Home Health encourages everyone to consider the impact they can make by becoming sustaining donors and fostering a sense of community support. Together, we can make a difference and bring comfort to those who need it most.

Holiday Blues Grief & Loss Support Group November 21st, 2023

November 1, 2023

Pemi-Baker Hospice & Home Health Starting Free Holiday Blues Grief & Loss Support Group November 21, 2023

Here in the United States, it seems like “the holidays” are now starting at Halloween. But as Thanksgiving approaches, the black cats, witches, ghosts, gravestones, and skeletons have to exit to make room for the turkeys, even though the pumpkins, gourds, and autumn leaves remain. But look! Behind them are the tree decorations, wreaths, ornaments, lights, snowmen, and wrapping paper!

The colors, sparkle, and glitz of the holidays, however, can leave persons unfazed who have endured losses of any kind. Someone will be missing from the Thanksgiving table this year. ‘Remember how Dad used to lose it and swear when hanging the lights outside?’ ‘We just moved here from Oregon in August and we don’t really know anyone yet.’ ‘Who’s going to make the potato latkes for Hanukkah now that Bubbe is gone?’ ‘I got laid off three weeks ago and haven’t been able to find work.’ ‘We had to put Princess down- she was so sick.’ ‘What’s New Year’s going to be like without Uncle Jack to snowmobile with or to watch the games on TV?’ The season of celebration and merriment can be an emotionally difficult time for those unsettled and disturbed by bereavement.

As in previous years, Pemi-Baker Hospice & Home Health will be offering its “HOLIDAY BLUES Grief and Loss Support Group” for those in need of processing various losses and in need of support in the company of others.

The group will begin on Tuesday, November 21, 2023 and will go for six sessions, concluding on Tuesday, December 26, 2023. The group meets at 10:30am in the upstairs art room (accessible by elevator) at the Plymouth Senior Center. The group process will include such handouts as “A Holiday Grievers Bill of Rights” to stimulate conversation and sharing as well as ideas for simple rituals that can be taken home. We strive toward providing an emotionally safe space where persons can find understanding and strength.

Persons who wish to participate must pre-register by contacting Guy Tillson, Hospice Chaplain and Bereavement Coordinator by email at gtillson@pbhha.org or by phone at (603) 536-2232.  Please contact him BEFORE Wednesday, November 15, 2023, so we have an idea of how many to prepare for.

National Home Health and Hospice Month: “Courageous Conversations”

November 1, 2023

National Home Health, Hospice and Palliative Care Month: “Courageous Conversations” – Pemi-Baker Hospice & Home Health Advocates for Open Dialogue.

November is National Home Health, Hospice and Palliative Care Month, and Pemi-Baker Hospice & Home Health is joining the nationwide effort to raise awareness and promote “Courageous Conversations” about end-of-life care. This year’s theme emphasizes the importance of initiating open dialogues within your inner circle of friends, family, and care providers about your healthcare preferences and choices. These conversations can be a powerful way to ensure that your wishes are respected and honored.

In a world that often shies away from discussing topics related to mortality and end-of-life decisions, “Courageous Conversations” encourage individuals to break the silence and engage in meaningful discussions. Pemi-Baker Hospice & Home Health believes that starting these conversations is an act of love and compassion, benefiting both you and your loved ones in the long run.

Initiating such discussions can be daunting, but it is an essential step toward ensuring that your values and preferences are understood and respected. Here are some suggestions for starting “Courageous Conversations”:

  1. “I know this isn’t a concern for us now, but I was thinking…” – This gentle approach allows you to express your thoughts without making it an immediate issue, giving your loved ones time to process and respond.
  2. “Remember when our (name of friend or family member) was sick/in the hospital? I’m wondering what you would have wanted if it were you.” – Relating your question to a shared experience can help open the conversation and make it feel less intimidating.
  3. Review and Update Advance Directives – Ensure your healthcare directives accurately reflect your current wishes and values. It’s crucial to keep these documents up-to-date, especially if there have been changes in your life circumstances or health status.
  4. The 5 D’s: Recognize that certain life events, such as Divorce, Death, Diagnosis, Decline, or Decade (significant milestone birthdays), can trigger the need to revisit your healthcare decisions. Use these events as reminders to update your preferences and inform your loved ones.

Having clarity about what is important to you in terms of your medical care can significantly ease the burden on those who may need to make decisions on your behalf. It provides them with the reassurance that they are acting in accordance with your wishes.

Pemi-Baker Hospice & Home Health offers support and guidance for those who want to initiate “Courageous Conversations” with their loved ones. Our team of healthcare professionals is available to provide information, resources, and advice on advance care planning and end-of-life decisions.

This National Home Health, Hospice and Palliative Care Month, we encourage everyone to take the first step in having “Courageous Conversations” about their healthcare preferences. By doing so, we can ensure that each individual’s dignity and choices are respected, providing peace of mind for both patients and their families.

This event is open to the public and will take place on Wednesday, November 15th, from 4:30 PM to 5:30 PM at the Boulder Point Community Room of Speare Primary Care at Health Place, located at 103 Boulder Point Drive, Plymouth, NH. Caregiver a love story showing and question and answer discussion with Pemi-Baker Hospice & Home Health, Plymouth, NH

‘Caregiver, a Love Story’ is a poignant 25-minute documentary that delves into the life of a 59-year-old woman named Bambi and her devoted husband, who becomes her primary caregiver after Bambi chooses to discontinue further cancer treatments. The film provides an intimate and heartfelt look at the challenges and moments of tenderness that Bambi and her husband experience as they confront the reality of her impending mortality.

Susan Varnum, Pemi-Baker’s Home Care, Palliative Care & Hospice Director, expressed her enthusiasm for the event, stating, “This film gives us a realistic glimpse inside what home hospice looks like and can initiate discussions we have now with ourselves and others about what matters most to us if we were in Bambi or her husband’s shoes. Our expert panel, which includes our Hospice Medical Director, will be available to answer questions related to hospice care and what matters most to you.”

The film screening and question and answer (Q&A) session are open to anyone interested in learning more about the invaluable services offered by hospice care providers and the profound impact it can have on the lives of patients and their loved ones. It’s an opportunity to gain insight into the challenges, emotions, and ultimately the love that often defines the caregiving journey.

Attendees are encouraged to ask questions, share their own experiences, and engage in meaningful conversations about the importance of end-of-life care and the choices individuals make during such critical moments in their lives.

Raising Awareness for National Breast Cancer Month

October 13, 2023

October is National Breast Cancer Awareness Month, and Pemi-Baker Hospice & Home Health is dedicated to spreading awareness about the importance of self-checks for both men and women. Breast cancer affects thousands of lives every year, and early detection through self-examinations can make a significant difference in the outcome. We urge everyone to take a moment to learn how to perform self-checks and understand the signs that should be discussed with their healthcare providers.National Breast Cancer Awareness Month

Breast cancer is not exclusive to women; it can affect men as well. Although the incidence in men is lower, it’s essential for all individuals to be aware of the signs and symptoms. Here are steps to perform a breast self-exam:

  1. Choose a consistent time: Select a time each month, preferably a few days after your menstrual cycle or on a specific date, to perform your breast self-exam. Consistency will help you notice any changes over time.
  2. Stand in front of a mirror: Examine your breasts visually, with your arms at your sides and then with your hands on your hips. Look for any changes in size, shape, or skin texture. Note any changes in your nipples.
  3. Raise your arms: Raise your arms and check for the same changes as mentioned in step 2.
  4. Lie down: Lie down on your back with a pillow or folded towel under your right shoulder and your right arm behind your head. Use your left hand to feel your right breast in a circular motion. Start from the outside and work your way toward the center, checking for lumps, thickening, or changes in texture.
  5. Switch sides: Repeat step 4 for your left breast, using your right hand.
  6. Check your armpits: Be sure to feel the lymph nodes in your armpits as well. Gently press your fingers into the tissue to check for any swelling or unusual bumps.
  7. Nipple discharge: If you notice any unusual discharge from your nipples, consult your healthcare provider.

Signs to discuss with your healthcare provider include:

  • New lumps or changes in the size or shape of your breasts
  • Unexplained pain or tenderness
  • Skin changes such as redness, dimpling, or puckering
  • Nipple changes, including discharge or inversion
  • Swelling or lumps in your armpit

Early detection is crucial in the fight against breast cancer. Regular self-exams, in addition to mammograms and clinical breast exams, are essential for early diagnosis and better treatment outcomes. Remember that breast cancer can affect anyone, so it is vital for both men and women to be proactive in their health.

As we take this opportunity to raise awareness about the crucial role of physical therapy in improving lives, we extend our heartfelt gratitude to our dedicated team of home health physical therapists who make a profound impact on our patients’ well-being.In home physical therapy from Pemi-Baker Hospice & Home Health, Plymouth, NH

This year, we are delighted to recognize the remarkable contributions of our outstanding physical therapists:

  1. Ernie Roy, PT, DPT
  2. Lauren Youngs, PT
  3. Judy Bolger, PT

  5. Stacey Barney, PT, DPT
  6. Pam Bello, PTA

These healthcare heroes have gone above and beyond in providing compassionate, personalized care to our homebound patients, helping them regain strength, mobility, and independence in the comfort of their own homes.

Physical therapy plays a pivotal role in addressing a wide range of health conditions, and our expert team at Pemi-Baker Hospice & Home Health is equipped to provide comprehensive care tailored to individual needs. Home health physical therapy can assist patients in managing and improving various health conditions, including but not limited to:

  1. Orthopedic Injuries: Our physical therapists are adept at helping individuals recover from fractures, joint replacements, and musculoskeletal injuries, enhancing their mobility and reducing pain.
  2. Neurological Disorders: We specialize in providing therapy for patients with conditions like stroke, Parkinson’s disease, multiple sclerosis, and spinal cord injuries, promoting better functional outcomes.
  3. Cardiopulmonary Conditions: Our skilled therapists work with individuals suffering from heart and lung conditions, enhancing their cardiovascular fitness and pulmonary function.
  4. Balance and Falls Prevention: We focus on improving balance, gait, and coordination to minimize the risk of falls among our elderly patients, ensuring their safety at home.
  5. Chronic Pain Management: Our holistic approach to physical therapy aims to reduce chronic pain and improve the quality of life for individuals dealing with ongoing discomfort.
  6. Post-Surgery Rehabilitation: We facilitate post-operative recovery, helping patients regain strength and mobility after surgical procedures.

It’s important to understand what “homebound” means. A person is considered homebound when they have a condition (medical, psychiatric, or social) that makes it extremely challenging or medically inadvisable to leave their home. This status is evaluated by a healthcare professional and is a crucial criterion for eligibility for home health services like physical therapy.

We invite everyone in our community to contact Pemi-Baker Hospice & Home Health to learn more about how our home health physical therapy services can benefit you or your loved ones. Our dedicated team is here to provide support, education, and compassionate care. You can reach us at 603-536-2232.

As we celebrate Physical Therapy Month, we extend our deepest gratitude to our exceptional team of physical therapists at Pemi-Baker Hospice & Home Health. Your unwavering commitment to improving the lives of our patients is an inspiration to us all.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.

You give of yourselves day & night without end!

To the Pemi-Baker Hospice Team--Words really can't express my feelings-

Your help during one of the most difficult times of my life is SO appreciated.  I'm so grateful that because of you, Doug was able to get his wish not to spend the last of his days in a hospital, but at home with those of us who loved him.

And to be constantly reassured that if I needed anything you would be there-oh my gosh-ANYTIME!  Well, you people need to know how much you are appreciated.  You give of yourselves day & night without end!  God Bless you and thank you!

“My home physical therapy was fantastic.”

Pemi-Baker provided very good home therapy. All their therapists were pleasant, helpful in offering suggestions for better functioning after surgery, and practiced good health protocols. They were also very prompt in showing up for scheduled appointments.

“Your generosity with your time and your loving care is appreciated more than you know.”

Thank you to everyone who played a role in taking care of my mother and giving her the freedom of remaining home while suffering from dementia. Your generosity with your time and your loving care is appreciated more than you know. A special thanks goes to Macayla and Colleen, who visited her on a regular basis, and to Lauren who was helpful in connecting us with state services.

“Pemi-Baker Hospice services provided kind, professional, competent services every step of the way-and even vaccinated us all for Covid/19!””

Our primary nurse, Fred, was outstanding, and others who filled in (Cathy, Penny, Colleen) were wonderful and a credit to your organization. Fred, you were amazing, not only for mom but for my husband and I as well. I will forever be grateful for your support, wisdom, humor and kindness, all along the end of life journey for mom. I am so deeply thankful that circumstances conspired such that mom was able to die at home, and your role made that time as seamless as it could be made.

Your courage during Covid-19 does not go unnoticed

Hello. We just wanted to take a moment and let you know how much we truly appreciate all that you do for us in these trying times. You're commitment to the health of our community is a special thing, and we know that you are working under stressful conditions due to the Covid 19 crisis. We realize that in order for you to treat those who may be ill, you are putting you're own lives and even you're own families lives in danger, and that is truly a courageous and honorable thing to do. We want you to know that it does not go unnoticed. Thank you all for all that you do!
